***Pre-School & Kindergarten Registration for the 2024-2025 School Year***
Registration is only open to students who are domicile in the Somerdale Park School District.
Registration Procedure for Pre-School and Kindergarten Students:
All children age four (4) by October 1, 2024 are eligible for our Pre-School Program.
All children age five (5) by October 1, 2024 are eligible for our Kindergarten Program.
Pre-School Registration for the 24-25 school year: Thursday 5/2/24 and Friday 5/3/24 (LIMITED SPOTS)
Kindergarten Registration for the 24-25 school year: Thursday 5/9/24 and Friday 5/10/24
Appointments will be between the hours of: 9:00 AM to 12:00 PM and 1:30 PM to 2:10 PM (every 20 minutes).
All registration forms are listed below for you to print and fill out at home before your appointment.
Please remember to bring all your required documents when registering your child.
Reminder, if you are missing any documents/information you will NOT be able to complete the registration process.
**PLEASE NOTE THAT PRE-SCHOOL HAS A LIMITED NUMBER OF OPENINGS AND AFTER
THEY ARE FILLED, STUDENTS WILL BE ADDED TO A WAITING LIST.**

(1st-8th graders) Please note that you can register your child during the summer months to start in September.
The following documents are required at the time of registration:
1. Original Birth Certificate (Copies or emailed copies will not be accepted) (ALL Students)
2. Medical Information:
A. Completed Medical Examination Form (must be less than 365 days old) (ALL Students)
B. Current Immunization Record (Please keep in mind these are required by law) (ALL Students):
Must meet the following requirements:
***For Kindergarten Registrants:
- DTaP Series - minimum 4 doses; one dose must be on/after 4th birthday
- Polio Series - minimum 3 doses of OPV or IPV; One dose must be on/after 4th birthday
- Measles/Mumps/Rubella (MMR) - 2 doses required; one must be after 1st birthday & doses must be separated by at least 28 days
- Hepatitis B Series - All Children born on/after January 1st, 1996 & entering Kindergarten are required to have received 3 doses of Hepatitis B vaccine.
- Varivax (chicken pox vaccine or proof of disease by physician)
- Proof of receiving Mantoux/PPD
***For Pre-School Registrants: (In addition to above)
- Haemophilus Influenza Type B (HIB) - All children 59 months of age or less are required to have at least one dose of HIB.
- PCV (Pneumococcal Conjugate Vaccine)- All Children 59 months of age or less are required to have at least 1 dose of PCV on or after their 1st birthday
- Influenza Vaccine (Flu) - All Children 59 months of age or less must annually receive at least 1 dose of Influenza Vaccine between September 1st & December 31st of each year & submit proof of receiving Vaccine to School nurse
C. Completed Confidential Health History Form (ALL Students)
D. Dental Form (ALL Students)
E. Lions Club Eye Screening Form (Pre-School and Kindergarten Students Only)
3. ELL Home Language Survey (ALL Students)
4. Proof of Residence/One (1) of the following: (ALL Students):
- Current Mortgage Statement/Mortgage Settlement Papers/Deed -OR-
- Current Apartment Lease (All occupants need to be listed) -OR-
- (UPDATED 3/12/24) Notarized Affidavit (if residing with someone else) along with copies of all documents provided to the notary public when affidavit was notarized & current drivers license (This person needs to show up in person at school registration appointment.) KEEP IN MIND THIS DOCUMENT NEEDS TO BE RENEWED AT THE BEGINING OF EVERY SCHOOL YEAR.
AND TWO (2) of the following PLUS Driver's License reflecting Somerdale address:(Confirmation of address change)
- Current utility bill (water, gas, electric, sewer, cable, etc. )
- Current tax bill
- Current homeowners insurance bill
- Current bank statement
5. Court Orders for Custody/Guardianship, State Agency Agreements/if applicable

***If you are transferring into Somerdale Park School, you can also use the following documents for registration along with supplying a transfer card, current report card and IEP (Individualized Education Plan)/504 Plan Section 504) if applicable.
